These are the official forms for use in Surrogate’s Court proceedings. The forms are arranged by type of proceeding. For many of the proceedings, a “checklist” is available on the related forms page. These checklists are provided to assist members of the public in completing petitions for common proceedings in the Surrogate’s Court, and should not be submitted to the court.
